Waiver / Liability Release Form
This is the form every member must have. (This form has been updated for all those who have already signed up with the old form)
Please ensure that the date of birth is the birth date of the actual member signing **for parents wanting to sign their children up.
It is important to note that the Waiver is only permitted to be signed by members that are 18 years of age or older.

The Liability form is the paper that is only to be filled out by the parent/guardian of the participant wanting to register.
n short the form ensures that the parent/guardian understands and acknowledges that they are taking part in a physical sport, and that preventative measures are taken place to guarantee their child's safety as long as they abide by the guidelines.
